Background
All recent national research, local research and consultation into services required by families of disabled children has revealed a need for greater inter-agency co-ordination. Parents often say that dealing with the helping agencies is the most difficult part of bringing up a disabled child.
In Pembrokeshire it was decided to start a co-ordination Keyworker service as from April 2001. Each agency contributes workers’ time and the project is managed by ACTION FOR CHILDREN. The project has been independently evaluated by the Social Policy and Research Unit of York University.
Eligibility Criteria
Children must be resident in Pembrokeshire and be between the ages of 0 to 16 at the time of referral. They must have a severe or ongoing disability and receive or be likely to receive services from more than two agencies. Referrals can be made by any agency.
What will my Key Worker do?
Having got to know your whole family, your Key worker will work with you to co-ordinate services from all agencies, provide you with information, assist with appointments, offer ongoing emotional support and can also work as an advocate for you.
If you feel the Key worker service would benefit you, please contact your health professional i.e. Health Visitor/Paediatrician etc who can make a referral.
